NEEDS ASSESSMENT

WHY Have a Needs Assessment

To find out what you need to continue to live in the community.

The person who will meet with you is called a Needs Facilitator and they will find out about you and your needs.

When the assessment is finished, another person who is a Service Co-ordinator will try to put together support to help you meet your needs and goals.

HOW Will it Happen

A “Needs Facilitator” (person) will visit to see what you or your family needs.

The visit is to find out, by working with you, what you can do and what you are good at. The person will ask you what you would like to do and what you need help with.

The facilitator (person) writes a list of your goals and what you need.

This is your “Needs Assessment Report” which belongs to you and you will be given a copy of it.

The report is private. You will be asked to sign a Consent Form to say who LifeLinks can show the report to. You can say who these people are.

You will be given time to read your report. When you are happy with it you will need to sign it to say you agree.

WHAT is Service Co-ordination?

Your “Needs Assessment Report” will be given to a Service Co-ordinator who will contact you and write a plan for you called a “Support Mix Plan”.

The Service Co-ordinator’s job is to:

  • Know about the many different services that can help you.
  • Help you to look for services that will work for you and/or your family.
  • Try to put you in contact with the services you need.
  • Contact you to see if services are OK.
  • Identify areas where no existing support services are available.

Please Note: Sometimes LifeLinks are not able to help with some of your needs because there is no help available
