Through our processes, we strive to ensure our clients are all enabled in the community. We listen and learn about you and your goals so that we have a better and empathetic understanding of your individual needs as well as those of your family.
With you we will plan and coordinate the community supports so that you can begin living your life the way you choose. We have a strong network of community relationships. These are pivotal to us delivering better outcomes for you and the community. We are committed to working alongside and with you. To do this we need you to be committed to working positively and proactively with us. We will work hard to create a positive non judgmental environment that will make it easy for you to talk to us.

What is Needs Assessment?

On acceptance of a referral we allocate a Needs Facilitator who contacts the client directly to arrange a time, date and place for the assessment to take place. There is no cost to the individual for this service.

This is a client driven/focused process. It proceeds at the client’s pace at the place of their choice. At the end of the needs assessment a report is prepared, the result of which is a summary of prioritised needs. The Needs Assessment process ends when the report is agreed to by the client, or the client’s representative. It is then signed off and a copy is provided to the client. A copy is also given to a LifeLinks Service Coordinator to begin the next process of Service Coordination

What is Service Coordination?

The Service Coordinator allocates the appropriate services to endeavour to meet the needs of the client. Service provider options are offered to the client (where choice is available) who will make the final choice as to which provider they want. The Service Coordinator then makes the referral, establishes a commencement date, and continues to monitor and review the services.

Our Kaumatua, Kawiti Tukuru, assists us to provide a culturally appropriate service. For other cultures we can access others to assist. E.g.: Refugee and Migrant Service, Pacific Island Trust, Deaf Association of New Zealand, Royal New Zealand Foundation of the Blind, Advocacy Services and any other service of choice.
